Youth Ministry Indemnity Form

1. NO LIABILITY - YOUR RISK
ROSEBANK UNION CHURCH and its employees, teachers, volunteers, representatives and/or agents SHALL NOT BE LIABLE FOR ANY LOSS AND/OR DAMAGE IN ANY CIRCUMSTANCES WHATSOEVER arising from and in respect of my/our child’s death, injury, loss and/or damage to person and/or property, HOWSOEVER CAUSED AND HOWSOEVER ARISING ON AND OFF THE RUC PROPERTY.
2. INDEMNITY - NO CLAIM
I/WE hold harmless and INDEMNIFY ROSEBANK UNION CHURCH, its employees, servants, teachers, volunteers , representatives and/or agents, AGAINST ANY CLAIMS OF ANY NATURE, including costs and expenses in respect of my child’s death, injury, loss and/or damage to person and/or property irrespective of whether such death, injury, loss and/or damage to person and/or property may have been caused by an act or omission, WHETHER NEGLIGENT OR OTHERWISE, of any such employee, teacher or volunteer, representative and/or agent of Rosebank Union Church.
3. YOUR CHOICE
I/We acknowledge that I/WE HAVE SIGNED THIS INDEMNITY OF MY/OUR OWN FREE WILL AND VOLITION and that I/we have not been induced to sign this indemnity as a result of duress and/or any representation of whatsoever nature by any official, representative, agent, volunteer, teacher or employee of Rosebank Union Church.
4. I/We acknowledge and consent, in accordance with the Protection of Personal Information Act (or POPI Act), that my/our personal information may be used by Rosebank Union Church only for the purpose of communication I/we initiated via the above form. My/our personal information may be stored by Rosebank Union Church on a secure server of an independent service provider. I/we acknowledge that Rosebank Union Church shall provide me/us with access to our own information and remove and/or destroy my/our own information should we so request.
